About Us

Integrated Medical Card Clinic, Prof. LLC, is a nurse-practitioner owned clinic dedicated to evaluating South Dakota residents for medical marijuana cards to help alleviate symptoms related to debilitating medical ailments.  We believe in patient-centered care for our clients and our goal is to certify qualifying clients to give them the opportunity to use medical marijuana as an alternative or complementary therapy for life-altering medical conditions.

Qualifying Conditions

  • Cancer with related nausea, weight-loss, or pain

  • HIV/AIDS

  • ALS

  • Crohn's Disease

  • Epilepsy/Seizures

  • MS

  • Glaucoma

  • PTSD

Qualifying Symptoms

  • Cachexia or wasting syndrome (severe weight loss/malnutrition)

  • Severe, debilitating pain

  • Severe nausea

  • Severe and persistent muscle spasms

South Dakota Medical Card Process:

1. Consult an approved provider​, such as Integrated Medical Card Clinic, Prof. LLC

2. Provide documentation to support symptoms or diagnosis (medical or psychiatric records, or a letter from a medical or psychiatric provider)
 
3. Apply for medical marijuana card through the state (additional $75 fee)

 

Our Provider Consult Fees:

Initial Certification: $165
Card Renewal: $135
 
If you do not qualify for a card, you will not be charged for the consult.

Payment with cash or card due at time of appointment.  $75 fee to the state due before card can be issued.
